5-Why Analysis
I found this great site that outlines how to conduct a root cause analysis. It provides a simple method as outline my Jeff Bezo from Amazon. It showed how Jeff cared enough about his employees to take the time out to find out why the injury occurred. You can view the site by clicking here for the exercise that Jeff conducted.
The second part of this website provided a method on How to apply the 5-Why concepts that entitled “Hacking at the Branches or Striking at the Root.” I really like what this author has put together. He uses a several illustration of the fish bone to demonstrate the 5-Why concept. Click here to view the website.
