Stucco Contractor Cited for Fall Hazards in 2007, 2008, and now 2009 — Occupational Health & Safety

OSHA has proposed $118,650 in fines against 4 Brothers Stucco Co., a Cleveland, Tenn.-based stucco contractor, for 15 alleged repeat and serious violations of safety standards, making this the third consecutive year the agency has cited the Cleveland, Tenn.-based stucco contractor. OSHA’s inspection found employees working on scaffolding, in an aerial lift, and on the roof at a worksite in Torrington, Conn., were exposed to falls of up to 22 feet. The inspection also identified electrical, overhead, and chemical hazard communication deficiencies at the worksite.

"These sizable fines reflect both the seriousness and recurring nature of several of the conditions cited here," said C. William Freeman III, OSHA’s area director in Hartford, Conn. "Keep in mind that falls are the number one killer in construction work and can occur in an instant. Be it a scaffold, an aerial lift, or a roof, proper and effective fall protection must be in place and in use at all times."

OSHA cites John Prouty Construction, Inc., following trench collapse that killed 4 in Verdel, Neb., Sept. 2008 – 04/1/09

OSHA has cited John Prouty Construction, Inc., of O’Neill, Neb., for alleged violations of the Occupational Safety and Health (OSH) Act following an investigation of a trench collapse near Verdel, Nebraska, that killed four workers, on Sept. 12, 2008.

OSHA’s investigation of the excavation company’s site found three alleged willful and two alleged serious violations of the OSH Act.

"There is no excuse for this accident and these workers did not need to lose their lives. It is appalling to realize there are companies that would allow, or even require, their employees to enter excavations without having cave-in protection," said Charles Adkins, OSHA’s regional administrator in Kansas City, Mo. "It is imperative that employers take the necessary steps to eliminate hazards and provide a safe working environment for all of their employees to prevent accidents like this from occurring."

Contractor cited following fatality at Miss. power plant

The U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) has cited LandCoast Insulation Inc. for three alleged safety violations following six injuries and one employee fatality last November when a scaffold collapsed inside a boiler at Mississippi Power’s Plant Daniel in Moss Point, Miss.
OSHA is issuing one willful violation with a $63,000 proposed penalty for substituting weaker, horizontal scaffold components in 14 locations. The agency defines a willful violation as one committed with plain indifference to or intentional disregard for employee safety and health.

Allowing workers inside an 11-foot-deep trench without safe egress

Vidalia, GA (CompNewsNetwork) – The U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) is citing McLendon Enterprises Inc. of Vidalia with six workplace safety violations and proposing $124,250 in penalties.

The citations come after an OSHA compliance officer observed company employees working in a trench without cave-in protection at a jobsite in Hinesville, Ga., last September.

Worker dies after fall at Larkspur building site – Marin Independent Journal

A worker died Thursday after falling off the roof of a home under construction on a Larkspur hillside, authorities said.

The worker fell about 4 p.m. at 7 Palm Court, a cul-de-sac about a quarter mile east of Magnolia Avenue on Palm Hill. The man tumbled near the rear side of the structure, and a fire spokesman estimated the drop at about 30 feet.

OSHA Cites Contractor For Safety Violations Following Trench Fatality

After the box was removed, one worker was crushed

Tallahassee, FL (CompNewsNetwork) – The U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) has cited Tim’s Hauling and Tractor Service Inc. for safety violations following a trench collapse that resulted in a fatality last October at a worksite in the Tallahassee area.

The Havana, Fla., company is being cited for one willful violation with a proposed penalty of $49,000 for allowing two employees to continue working in an 8-foot-deep trench after removing a protective trench box. After the box was removed, one worker was crushed when a sidewall of the trench collapsed. The agency defines a willful violation as one committed with plain indifference to or intentional disregard for employee safety and health.

Construction worker hit by metal roofing

A Melbourne construction worker is lucky to be alive after being hit in the head by metal roofing that was blown from a building site by gusty winds.

The 46-year-old man, who was working on the building site at Dandenong South, suffered a cut to his head after he was struck down by the flying metal – which weighed several hundred kilogram – just after midday.
